A community health assessment is a process by which community members gain an understanding of the health, concerns, and health care systems of the community by identifying, collecting, analyzing, and disseminating information on community assets, strengths, resources, and the needs of our community.
The assessment will provide direction and focus for Scotland County Department of Public Health and Scotland Health Care System and other community agencies and organizations in Scotland County. This assessment will serve as a basis for improving and promoting the health of Scotland County residents.
